Fat Injection
Injection of patient's fat into breast used to be popularly operated in Europe and U.S.A. since it is believe to be safe. However, some cons appear:1. Fat preparation requires excellent skill and well sterilization. 2. This technique can be applied to only patient with enough fat. For example, if 100 cc. of prepared fat is required for injection into each breast, 300 - 400 cc. of fat needed.3. The injected fat will mostly reduce to 50% within 6-12 months and will be completely gone after 1-2 years.4. If the fat preparation is incomplete, fat may be solidified. Therefore, it will be difficult to identify whether it is the injected fat or tumor5. Patient with fat injection is not recommended for implanting silicone jacket since it will cause breast solidification. The best application is to inject removable gel like polymer.


Breast Enlargement
Breast enlargement using silicone jacket has been operated for almost 40 years and proved to be safe from causing cancer. The silicone jacket can stay for life if there is no implication such as leakage of silicone jacket or too much firm layer of tissue occurs.

2. Smooth
It is believe that the rough silicone jacket causes less tissue reaction than the smooth one. However, there is no scientific study to confirm the result.


Filler
1. Silicone gel is popular filler being used to fill in the jacket. The pros is its natural-like softness. The cons is if the jacket leak, silicone gel will intrude into surrounding tissue and develop a firm layer of scar tissue around the implants which harden the breast.
2. Saline solution is another filler. The pros is there is no harm from the jacket leakage. The cons is there are chances of jacket leakage. The operation can be done with local anesthetic and sedation or under general anesthesia which takes around 30-60 minutes.


Scars are an unavoidable result of the operation which can be found in 3 types:

1. Scar in the armpitThe pros is the scar will be hidden under the armpit The cons is the operation is more complicated
2. Scar around the areola (nipple edge)The pros is the operation is simple The cons is the scar is obvious
3. Scar under the breastThe pros and cons are same as the scar around the areola

Postoperative care
- Patient must stay overnight after the operation
- Avoid from getting wet for 7 days
- Sutures can be removed after 7 days
- Patient can be back to work within 3-5 days postoperatively
- Even pain is gone, patient should continue massage her breast for months to prevent the development of firm layer of tissue



Breast Reduction
Problems concern large, heavy breasts can be minimized or eliminated by breast reduction. The surgeon must carefully evaluate whether the patient experience several health concerns related to her breasts or not, for example, back pain. If the symptoms do occur, there are 2 solutions:
1. Liposuction around breasts area. This solution can make the breasts smaller but not quite obvious.
2. Surgery. This solution gives exact result but scars are an unavoidable drawback

Breast Uplift
Breast sagging is a common problem following aging and other causes like weight loss, pregnancies. Breast sagging can be classified into 3 class

Breast sagging can be corrected by surgery. The surgeon repositions the nipple, areola and breast mass to a higher position.
