The history of Microimplant injection technique or Macroplastic injection technique began about 30 years-ago by Dr. Planas; he sliced the silastic sheet to many tiny pieces and injected with syringe to correct the depress deformity of the chest wall (pectus excavatum) and also some facial rhytides and other depress skin deformities.
He collected the scientific datas for over 20 years and found the satisfied results without serious complication but he still cannot made the injected area to have the definite contour. In1998 , Dr. Ersek was collected the scientific datas of this technique in 12 medical schools in more than 2000 patients who has been treated in different areas all over the body and found that the technique works well but the surgeon has to be well-trained and has to understand the concepts of Microimplant injection technique.
Why Biotechnique?, because in those old day – cosmetic surgeons always use the autogenous tissue ( cartilage , bone etc.) but there too many problems like resorption . limitation of amount of tissue and the permanent surgical scar. Then the decade of polymer was came to the field of cosmetic surgery , many kinds of polymers (polymethylmethacrylate , Teflon) have been use for enhancements still there are some negative consequences ; allergic reaction, difficulty in contouring to desired shape. The silastic sheet was well-accepted in later date because its properties of inert , non-toxic and easier shaping to fit the enhance site but still high evidences of thining of the covered skin or even perforation , resorption of bony structure underneath the silastic sheet , deviation , infection and also risks of complication to surgery (trauma to major nerve or blood vessel , permanent scar).According to article of Dr. Takashi Endo , he collected datas from the 1,263 patients who have got the augmentation rhinoplasty compared between different materials (bone , cartilage and silastic sheet).
He found that 510 patients (40%) who got the augmentation rhinoplasty with the silastic sheet have to got the revision operations of their nose from the causes of the disproportion (too large or too high 74%) , mental condition 20%. Another 753 new patients got the problems about the shape of their nose are too short or too high with 4% of deviation and infection. (PRS. January 1991, Vol.87, Number 1: Augmentation Rhinoplasty)
In1985 there was a medical equipment company named Uroplasty, brought the principles of Microimplant injection technique to develop the finished product called “Bioplastique”.
The Bioplastique was distributed to all over the world and well-accepted to use in correction of vocal cord paralysis , urinary incontinence. There are many articles about Bioplastique published in standard journals – Plastic and Reconstructive surgery , Aesthetic Plastic Surgery etc. Dr. Rosert A. Ersek was introduced Bioplastique to chief of the plastic and reconstructive surgery in medical schools in Germany, Holland, Belgium, Switzerland, Austria, france, England, Norway, finland, Spain, Japan, Hongkong, Taiwan and Singapore
Up to 1992, There are more than 2000 patients achieved good results with Bioplastique, The Bioplastique is good for nose, chin, cheek, malar and dorsum of hand but surgeons have to avoid superficially injection but if it occur the correction can be done by micro– liposuction technique.
As we known form legend above, the Biotechnique or Microimplant injection technique was developed and modified from simple easy technique to the level of making a finished product with world wide distribution.
